I acknowledge that participation in flag football activities involves inherent risks of injury, including but not limited to cuts, bruises, sprains, broken bones, and other serious injuries. I understand these risks and voluntarily assume all risks associated with participation.

I hereby release, waive, discharge and covenant not to sue the flag football league, its organizers, coaches, officials, and facility owners from any and all liability, claims, demands, or causes of action arising from my participation in flag football activities.

I certify that I am physically fit and have no medical conditions that would prevent safe participation in flag football activities. I agree to inform organizers of any changes to my health status.
